ISOPP Conflict of Interest Disclosure Form


It is ISOPP’s policy for all ISOPP volunteers and contributors to disclose any real or potential conflict(s) of interest between their ISOPP service(s) / contribution(s) and their other activities.

ISOPP volunteers must disclose any conflicts on an annual basis, and must disqualify themselves from voting on any such matter and/or to remove themselves from service if an ongoing or new conflict exists.

Contributors to ISOPP must disclose any conflicts when the contribution is made, and if/when the contribution changes and/or the contributor's situation changes whereby either or both are in a conflict of interest. A declaration should be made: 1) if the contributor received financial support outside of his/her institution towards the preparation of the resource; 2) if there is/was a financial relationships with entities that could be perceived to influence, or that give the appearance of potentially influencing, the contribution; and 3) if there are any other relationships or activities that ISOPP members may perceive to put the contribution in conflict of interest.

For the purpose of this form, you need to declare if any conflicts exist with you personally or with a close friend, immediate family member (spouse or dependent child), business associate, corporation or partnership in which you hold a significant interest, or a person to whom you owe an obligation. We refer to these others as “personals affiliated with me.”
