Welcome to the online registration site for CAPhO Conference 2023 taking place from April 13 to 16 both virtually and in person at the Westin Harbour Castle in Toronto.

During the online registration process, you can register for the Conference and/or register for the BMT Workshop, and purchase guest ticket(s) for the CAPhO Awards Gala. Conference registrants will be sent an email once the satellite symposia, concurrent sessions and round table discussions are confirmed, giving you the opportunity to pre-register for those sessions.

2023 Bone Marrow Transplant (BMT 101) Workshop Registration

The Bone Marrow Transplant (BMT 101) Workshop will be held on Thursday, April 13, 2023 from 12:30 to 16:30 in conjunction with the Conference.

If the participant limit has been reached your name will be added to a waitlist and you will be contacted if space becomes available.

Cancellation, Refund and Substitution Policy

Conference and Awards Gala
Refunds for Conference registrations and Awards Gala guest tickets are available until Wednesday, March 8, 2023 and are subject to a cancellation administration fee of $100 per registration and $50 per ticket. As of Thursday, March 9, 2023, no refunds will be provided, however substitutions can be made until midnight PDT on Friday, March 31, 2023.  

 

Bone Marrow Transplant (BMT 101) Workshop
There are no refunds for cancellations; however, substitutions can be made until midnight PDT on Friday, March 31, 2023. 

 

All cancellations or substitution requests must be received in writing by the Association Management Office at conference@capho.org. Substitutions must be made at an equal or lesser registration category. No refunds will be issued for substitutions.

 

Individuals take part in the CAPhO Conference and/or Bone Marrow Transplant (BMT 101) Workshop at their own risk and are responsible for their own health and travel insurance arrangements.

Debit Card (Visa Debit, Debit MasterCard and Maestro) or Credit Card (American Express, MasterCard and Visa)

If paying by debit or credit card, CAPhO Conference or CAPhOConf will appear as the vendor on your debit or credit card statement.. If your debit or credit card payment is declined, select “Cancel” on the payment page and select "Invoice for cheque payment" as your payment method and then click on “Submit” to process your registration. You can log in and process the payment via a debit or credit card at a later date or pay via cheque.

Cheque

If you are paying by cheque, include CAPhO 2023 and the name of the registrant on the cheque, make the cheque payable to Sea to Sky Meeting Management Inc, and include a copy of the invoice when mailing the cheque. Funds are due in Canadian Dollars (CAD) and must be drawn from a Canadian bank. The payment address will be included on the invoice that is sent to the email address provided in this registration upon completion of your registration. A receipt will be sent by email once payment has been received.
