By submitting this application, the applicant agrees:
That the educational materials, programs or resources submitted comply with CAPhO’s Development of Education Policy & Procedure for Internal Member-Driven Initiatives.
That at least one submitting author is a current CAPhO member.
That the proposed materials relate to oncology pharmacy practice, bridge current learning gaps and include only Health Canada-approved, publicly reimbursed treatment options.
That the materials align with CAPhO’s mission and vision .
To having CAPhO assign a director from CAPhO Board of Directors as a liaison to facilitate the project.
To including CAPhO’s logo unaltered on all written materials, which will also display the date of production/publication.
That funding availability is determined annually by CAPhO’s Board of Directors and is subject to change.
That applicant(s) do not currently receive nor intend to apply for external sponsorship or grants for development.
That the review timelines (approximately 4-6 weeks) are determined by CAPhO and may vary.
That a Memorandum of Understanding (MoU) will be signed prior to project commencement.
That upon completion, CAPhO will make the materials accessible to members and the public; submitters may also disseminate the finalized materials.
That CAPhO reserves the right to remove outdated or irrelevant materials from its website.
To collaborate with CAPhO representatives throughout the development and review process.
